A $25 booking fee is required to book any treatment. The booking fee will go toward the booked treatment/appointment. The booking fee is non-refundable if the appointment is not cancelled at least 24 hours before scheduled appointment or you do not show up for your appointment.

Indy Elite IV Drip and Wellness does not accept any form of insurance. You are expected to pay in full at time of service. Indy Elite IV Drip and Wellness will not contact your insurance company nor will take any responsibility for providing claims to your insurance company.
